Please complete and submit this pre-course registration a minimum of 10 working days (Mon-Fri) prior to your course to allow for course administration. Where you require Reasonable Adjustments and/or Special Considerations you will be contacted by your tutor to discuss further prior to your course.

Photographic I.D. Declaration(Required)
In order to undertake the accredited training course you have enrolled on you MUST produce in-date photographic I.D. (passport or photo driving licence) in order to undertake this course. You understand that failure to provide satisfactory photographic I.D. will mean you will not be able to undertake the course. Your course would have to be rescheduled, with additional charges. No refunds will be provided for course fees where you fail to produce photographic I.D. as required.
What photo I.D. will you be bringing to the course?(Required)
Photographic I.D. will be required on starting the course as an identity check for your registered and regulated course. The valid photo ID can be a UK or international passport or photo driving licence. The photo I.D. MUST be in date and match your current legal name and/or address details you will be providing on the course registration at the start of the course. N.B.: If you do not bring valid photo I.D. with you on the first day of the course you will NOT be able to complete the course.
Photographic I.D. Name(Required)
Your current name MUST match that on the photographic I.D. you are producing. If the names DO NOT match you will not be able to complete the course.
Do you require Reasonable Adjustments for your learning?(Required)
To provide suitable arrangements for candidates who require Reasonable Adjustments to assessment arrangements as a result of a permanent or long-term disability, or temporary indisposition. You will be contacted prior to the course where a separate declaration must be made on the appropriate forms, for each candidate undertaking an assessment requiring Reasonable Adjustments. Reasonable adjustments authorisation must then subsequently be received by the examining board prior to the course.
Please provide brief details and your tutor will contact you to discuss further, to allow for the reasonable adjustments request to be submitted to the examination board for approval.
Do you require Special Considerations?(Required)
This may be applied to a candidate or group of candidates that has / have, through illness, injury or another event outside of their control immediately prior to or at the time of an assessment, been unable to take or complete an assessment or been unable to demonstrate their expected level of attainment. N.B. You will be contacted prior to the course as Separate declarations must be made on the appropriate form, for each candidate undertaking an assessment requiring. Special considerations authorisation must then subsequently be received by the examining board prior to the course.

Timekeeping and Course Attendance(Required)
In order to undertake the accredited training course you MUST attend the course at the start time provided and complete the course fully (not arriving late, departing midway or extending any allowed breaks. Where you fail to fully attend the learning hours of the course you will be withdrawn from the course; your employer/you would then have to re-book (at an additional charge) to fully complete the course.
Mobile Phones and other electronic devides(Required)
You MUST NOT use your mobile phone or other electronic devices during the course (except in cases of emergency), during the learning hours of the course or the examination. Mobile phones must be switched off and stored away from your working position during the course (for example in your personal belongings). You understand that where you are discovered using a mobile phone during your examination you will be withdrawn and will have to re-take your examination at a later date (with additional charges).
Watches/Smart Watches(Required)
You MUST NOT use your watch (including smart watches) during the examination at the end of the course. Watches, like mobile phones, must be removed and stored away from your examination area (further guidance will be provided by your tutor and exam invigilator at the time. You understand that where you are discovered using a watch/smart watch during your examination you will be withdrawn and will have to re-take your examination at a later date (with additional charges).
Statement(Required)
You MUST provide true and accurate information and documents. You MUST NOT make false statements, provide false information or provide false identity documents during course pre-registration or during the course. You understand that providing inaccurate or false I.D. documents may invalidate your course or qualification gained.
